Thank you for your interest in volunteering at Freedom Chapel. An application is required for all persons wishing to volunteer in Freedom Kids, Freedom Youth, GateKeepers, Worship Ministry, and Freedom Connects Group Leaders. A background check is ran on all volunteers in these departments as required by our church insurance policy. Upon review b church leadership the ministry director for the department you are applying for will contact you.

I authorize any references or churches listed in this application to giv you any information they may have regarding my character and fitness for children/youth work. I release all such references from liability for any damage that may result from furnishing such evaluations to you.

I understand that if I am accepted as a volunteer I agree to be bound to the Constitution, Bylaws and policies o Freedom Chapel. I agree to refrain from unscriptural conduct in the performance of my services on behalf of the church and in my daily life.
As part of the application process for volunteering at Freedom Chapel, I understand that they and/or its agents may conduct an investigation of my personal information. The investigation migh include, but is not limited to names and dates of previous/current employment, work experiences, workers' compensations claims, criminal history records (from state, federal, and other agencies), motor vehicle records, military records, names and dates of education, credit history, and bankruptcy records. I understand that these records may be used for eligibility to volunteer. I authorize without reservation the full release of these records to obtain information from all available resources throughout and duration as a volunteer at teabve stated company.
